SPlease bring the following documents with you when registering new students:
- Birth Certificate
- Social Security Card
- Kentucky Eye Exam Form
- Kentucky Physical Form (acceptable 12 months prior to the start of school)
- Kentucky Immunization Form
- Proof of Residence (Utility bill, lease, etc)
